چکیده
Abstract Objective To assess whether children undergoing tonsillectomy were receiving adequate analgesia postoperatively and comparison made against national standards. Implementation of new ibuprofen dosage guidance, reducing opioid prescribing, assessment this impacts re-admission rates for post pain bleeding. Method Retrospective audit tonsillectomies performed over a 5-month period in 2021 prospective the same time 2022, with prescribing guidelines (7.5mg/kg QDS compared 5mg/kg TDS previously) effect on reduced prescribing. Results There approximately 110 each cycle. For first retrospective cycle, 95% prescribed inadequate ibuprofen, mean dose 5.5mg/kg. All patients who readmitted pain/haemorrhage had at frequency. 6% cases presenting bleeding, none returned to theatre. Coblation was used about 40% (extracapsular intracapsular). second 55% have been correct 7.5mg/kg QDS, No re-admitted post-operative only low number presented post-tonsillectomy bleeds. Conclusions Changing dosing ensuring departmental consistency has resulted no admissions poorly controlled post-tonsillectomy. We feel secondary impact is bleeding rates.

Morphine or Ibuprofen for post-tonsillectomy analgesia: a randomized trial.
BACKGROUND Pediatric sleep disordered breathing is often caused by hypertrophy of the tonsils and is commonly managed by tonsillectomy. There is controversy regarding which postsurgical analgesic agents are safe and efficacious. METHODS This prospective randomized clinical trial recruited children who had sleep disordered breathing who were scheduled for tonsillectomy +/- adenoid removal. Par...
متن کاملEffect of Ketamine on Post-Tonsillectomy Sedation and Pain Relief
Introduction: Tonsillectomy is the one of the most common types of surgery in children, and is often accompanied by post-operative pain and discomfort. Methods of pain control such as use of non-steroidal anti-inflammatory drugs (NSAIDs), narcotics, and local anesthetics have been used, but each have their own particular side effects. In this study we investigated the effect of ketamine on post...

Dexamethasone administration prior to tonsillectomy has been shown to reduce morbidity and is part of SIGN guideline 117. We conducted a three-cycle audit of 149 patients to ascertain how well guidelines were being met and introduce a sustainable method to improve compliance. A 3-month audit was conducted to ascertain how many tonsillectomy patients didn't receive pre-operative dexamethasone. E...
